Claim Forms
  • Instructions for Claim Forms - This document provides step-by-step instructions on how to submit a claim form

  • Interim Claim Form - This form must be completed by the project proponent and submitted with the Detail Claim Form.  It includes financial totals from the Detail Claim Form and indicates the amount the proponent is expecting to receive from CNSIS.

  • Detail Claim Form - This form provides line-by-line details of each invoice submitted by the project proponent.  Invoices and cancelled cheques must be submitted with this form, and it must be submitted on a regular basis.  Do not wait until the completion of the project.

  • Project Audit Certificate - This form provides final financial information about the project upon completion of the project.  The project proponent must provide this form to their Municipal Auditor to be used when conducting the project audit.  The form must be signed by the Municipality and the Municipal Auditor before it is submitted to the CNSIS to receive the 10 per cent holdback.
